Additional MaximEyes Information for Trustees
Dear Ex Officio Trustees,
It is with much pleasure that APH introduces you to the MaximEyes Video Magnifier for customers with low vision. This product is unique with its PenTracker feature for improved writing capability and mouse control of the camera movement and magnification.
LIMITED QUOTA FUNDS AVAILABLE
You will note that this is the most expensive product that APH has ever introduced and made available with Federal Quota funds. At $3,495.00 each, no more than five (5) units will be available for purchase with Federal Quota funds per account annually. Of course, unlimited quantities will be available for purchase using cash, credit card, or other funding.
SPECIAL SPLIT PAYMENT OPTION
For those Trustees with limited funds, we are offering a split payment plan. At your option you may order a unit and forward a signed copy of the attached financial authorization (below) to take one-third of the price or $1,165.00 each year for the next three years. The first payment will be accessed at the time the product is delivered, the second and third payment will be drawn from you account on October 1 of the next two years. This is intended to allow you to budget your allocation and purchase additional needed products throughout the year. Please understand that this is an option. If you have the funds and wish to pay the full amount upon receipt, that is acceptable. Some Trustees have already indicated that they do not plan to use quota funds, but instead plan to utilize other funding available to them. (Orders using this option must be mailed or faxed.)
MaximEyes™ Video Magnifier Split Payment Authorization
I, , Ex Officio Trustee for Federal Quota Account Number , hereby authorize APH to withdraw $1,165.00 each over 3 years to pay for the purchase of a MaximEyes Video Magnifier.
It is noted that the first such payment will be charged along with delivery fees when the product is shipped. Each subsequent two charges of $1,165.00 each will be withdrawn on the first of October when new quota funds are available.
This authorization accompanies my Federal Quota Order Number .
