INVOICE INFORMATION

INVOICE INFORMATION REQUIRED:
- Your Name or Company Name, Address, Phone Number, Fax Number
- Date of Invoice
- Invoice Number
- Title of Book
- APH Catalog Number (A-B0???-00)
- Which volumes the invoice is for(1-3, Supp. 1-5)
- List the Volume Number and the amount of Braille pages in each volume, and the rate charged per page (should match your Cost Per Page on the ATIC Bid Request.)
- List the amount of graphics pages in the volume, and the rate charged per graphic page (Even if there are zero (0) graphics, please list the number zero (0).
- List the shipping cost(s), and include the receipt(s) (if applicable)
- List the Total Amount Due
- Include any receipts you have for material or shipping that you are charging us for (e.g.: trade of materials for transcription)
- State on each invoice "THIS DOES or DOES NOT COMPLETE THE BOOK". (We have no way of knowing when the end of the book has been done without this notification from you).
