Customer Satisfaction Survey 2018

Take the survey now!

Greetings Ex Officio Trustees, IRC Personnel, Users of APH Products,

Each fall APH is required to conduct a Customer Satisfaction Survey for the U.S. Department of Education. This survey is intended for those directly involved with APH, i.e., Ex Officio Trustees, but we are also seeking input from professionals in education and rehabilitation, students, consumers, and their family members to determine the effectiveness of our products and services. The survey is just 11 questions and should only take a few minutes of your time. It will allow you the opportunity to rate your satisfaction with our products and to express your thoughts and experiences with them. Your feedback will provide valuable input both for the Federal Quota Program and for the staff of APH.

Deadline for the survey is October 8, 2018.

Protein Synthesis Kit
The Protein Synthesis Kit is example of the hundreds of products offered by APH. This kit is an accessible, interactive model that adds to the molecular biology concepts introduced by the DNA-RNA Kit.

If you are an Ex Officio Trustee of APH, your participation in this annual survey is critical. The federal GPRA performance measures and the Dept of Ed require us to report separate response data for 1) the full pool of Ex Officio Trustees, 2) the Trustee Advisory Committees, 3) Teachers of the Visually Impaired, and 4) Other Consumers. We need a response from you because of your vital role as an Ex Officio Trustee of APH. Please have your Federal Quota Account number(s) at hand when you submit your responses on line.

What if you do not personally use APH products frequently? It is acceptable for your responses to reflect the experience of professionals who teach students or clients registered through your account. It is also acceptable for you to respond only to the questions that you feel you are able to answer given the situation with your Federal Quota Account. This survey is open to anyone who has used APH products in educational or rehabilitation settings. We encourage you to share this opportunity for input with your staff, other professionals, and all appropriate parties.

We sincerely appreciate your sharing your time and expertise with us. You can respond to the survey by going to the link
